Medication Administration Authorization Information


With the cold weather comes illness and coughs. Please remember that students are not allowed to have ANY kind of medication (over-the-counter or prescription) with them. This includes chap-stick, cough drops, and inhalers. If your student needs to take medication at school, you must fill out an authorization form and have it signed by the doctor. All medication must be transported to school by an adult and be contained in the original packaging with prescription or dosing label (for over-the-counter medications).
